Why a Utah License Matters in Salt Lake City
Licensed where you rent
The clinician who evaluates you holds a Utah license, the state your lease is in.
Honest answers count
Misleading a health care provider to get animal paperwork is a class C misdemeanor in Utah.
Federal test, local lease
Utah relies on federal accommodation rules, so the letter has to describe a disability-related need.
